What to Feed a Dog With Kidney Disease

The thing that matters most is phosphorus, not protein. That surprises most owners. International guidelines for dogs call for cutting phosphate as the disease moves along. They do not call for cutting protein at any stage. The stage matters enormously, and Stage 1 asks for much less than people expect.

The one thing most owners get told wrong

"Kidney disease means low protein" is the advice almost everyone hears, and it is an oversimplification that has aged badly.

The International Renal Interest Society guidelines for dogs, which are what most vets work from, set out a staged approach built around dietary phosphate restriction as the disease progresses. Reading the canine document through, protein restriction is not recommended at any stage. Protein appears in the context of urine protein and managing protein loss, which is a different issue.

That is not a fringe reading. It matches the cleanest experiment ever run in this area.

What the research found when it separated the two

In a 24-month controlled feeding study, 48 dogs were assigned across a design that varied phosphorus and protein independently: high or low of each, in all four combinations. It is the only study that cleanly separates the two variables.

Survival was significantly improved by the low-phosphorus diets. Dietary protein had no significant effect on survival, or on how long kidney function remained stable (Finco et al., 1992).

A later double-masked randomised trial of 38 dogs confirmed that a renal diet beats ordinary adult food on uremic crises, mortality, and the rate of decline in kidney function (Jacob et al., 2002). But it is worth being precise about what that shows: the renal food differed in protein, phosphorus, sodium, omega-3, and buffering capacity all at once, so the trial cannot tell you which change did the work. Read alongside Finco, the phosphorus reading is the better-supported one.

There is also a broader point about protein worth knowing, particularly for older dogs. Reviewing the evidence, one veterinary nutritionist summarised it directly: numerous studies have confirmed that protein does not adversely affect the kidneys, and restricting protein in healthy older dogs is not only unnecessary but can be detrimental (Laflamme, 2008). Healthy senior dogs need more protein, not less. That is a separate question from a dog with established disease. But it is why "old dog, cut the protein" is bad advice on its own.

Does high-phosphorus food cause kidney disease?

Different question, and the honest answer is that in dogs it has not been shown.

A case-control study compared 75 dogs with chronic kidney disease against 57 age-matched controls, looking at what they had been eating before diagnosis. In dogs there was no difference in prior phosphorus or protein intake between cases and controls. The positive signal (higher pre-diagnosis phosphorus intake) appeared only in the cats in the same study (Böswald et al., 2018).

So phosphate control is well supported as treatment once disease is established. It is not established that dietary phosphorus causes canine kidney disease. Those two things get conflated constantly, usually to sell something.

What the guidelines actually say by stage

Roughly, and simplified (your vet works from the full document and your dog's blood results):

The practical detail buried in there is worth pulling out: the guidance suggests introducing a renal diet early, because it becomes much harder once a dog has lost their appetite. If your vet raises it at stage two and your dog is still eating well, that timing is deliberate.

Where AlphaPak fits, and where it does not

The answer genuinely differs by stage, and that is worth spelling out rather than flattening.

Stage 1: no restriction is being asked for

At Stage 1 the guidelines set no phosphate target and do not ask for a renal diet. That only comes up if the dog is losing protein in their urine. As for protein, the evidence is clear: it does not damage healthy kidney tissue. The one study that separated protein from everything else found no effect on survival at all.

So a dog with Stage 1 disease, not losing protein in their urine, is not being asked to restrict anything yet. There is no evidence-based reason our food is unsuitable for that dog, and if they are eating it happily, that is worth something. Appetite gets harder later, not easier. Confirm it with your vet, who has the bloodwork.

Stage 2 onward: a renal diet is the intervention

From here the honest answer changes, and it is a no:

  • Both recipes contain bone meal, a concentrated source of calcium and phosphorus. Phosphate restriction is the whole intervention. We run the other way.
  • We have not had phosphorus assayed, so we cannot give you the number. In a conversation where the number is the entire point, that alone should keep us out of it. Getting that analysis done is on our list.
  • Protein is high, at roughly 112 to 114 grams per 1,000 calories against 30 to 45 in a renal diet. Not the primary lever on the evidence above, but not nothing in advanced disease.

One point is genuinely in our favour, though it does not change the Stage 2 answer. Research in dogs suggests phosphorus from bone is gentler on the body than added phosphate salts at the same level. The form does matter. But the guidelines ask for less phosphate, not better phosphate.

One further caution at any stage: dogs with kidney disease are sometimes on immunosuppressive medication, and a raw-format food is not a good pairing with that.

What to actually do

Ask your vet which stage your dog is at, and what the blood phosphate target is. Ask whether a renal diet makes sense now, or is worth starting early. And if the real problem is that your dog will not eat (which it very often is), ask about anti-nausea and appetite medicine before you work through more foods. A renal diet your dog actually eats beats a perfect one they turn down.

And if your dog has very early disease with no phosphate target set, ask specifically what that means for their food. The answer may be less restrictive than you expect.

Common questions

What should I feed a dog with kidney disease?
A therapeutic renal diet chosen by your vet, in most cases from the point the disease is more than mild. These diets are built around restricted phosphorus alongside other modifications. Getting a dog with reduced appetite to eat is often the hardest part, so involve your vet early rather than switching foods yourself.
Do dogs with kidney disease need low protein?
This is more nuanced than the usual advice suggests. The international guidelines for dogs recommend dietary phosphate restriction as the disease progresses and do not recommend protein restriction at any stage. In a two-year controlled trial, survival was improved by the low-phosphorus diets while dietary protein had no significant effect. Your vet still sets the target, because advanced disease and other complications can change it.
Is high phosphorus bad for dogs with kidney disease?
Phosphate control is the best-supported dietary intervention once kidney disease has progressed, and the guidelines set specific blood phosphate targets by stage. That is a different question from whether high-phosphorus food causes kidney disease in the first place. In dogs, one case-control study found no difference in prior phosphorus intake between affected dogs and controls.
Can a dog with kidney disease eat raw food?
A bone-inclusive raw diet is a poor fit, because ground bone makes it phosphorus-dense, the opposite of what a renal diet does. There is also an infection consideration if the dog is on immunosuppressive medication. This is a decision for your vet, not a website.

This is the real day-to-day problem, and it is worth raising with your vet directly. Reduced appetite and nausea are part of the disease, and there are anti-nausea and appetite medications that help. Warming food, smaller more frequent meals, and starting the renal diet early, before appetite deteriorates, all make it more likely to be accepted.
